Shoulder pain: rotator cuff, tendinopathy and frozen shoulder — what the evidence actually says

If you’ve been told you might have a “torn rotator cuff,”
you’re probably searching for one thing: can this heal without
surgery?
You’ve likely already found the videos — some
genuinely good exercise programmes, some promising a quick fix, a few
implying that most physiotherapists are getting it wrong. This page
answers the surgery question honestly, using real numbers, not a sales
pitch in either direction. It also covers the shoulder problem that gets
confused with a cuff tear but is a completely different condition:
frozen shoulder.

Rotator cuff tear, tendinopathy, impingement or
frozen shoulder? Why the label matters

“Shoulder pain” covers genuinely different problems, and they don’t
get the same treatment. A useful way to sort them, broadly:

  • The stiff shoulder — most often frozen
    shoulder (adhesive capsulitis)
    : progressive loss of
    both active and passive movement, often with no clear trigger.
    It behaves and is treated very differently from the other two categories
    below, so it gets its own section further down.
  • The unstable shoulder — related to a history of
    dislocation or subluxation (the joint slipping partly or fully out of
    place). Different mechanism, different rehab priorities.
  • The weak and painful shoulder — the umbrella for
    shoulder impingement, subacromial pain syndrome, rotator cuff
    tendinitis/tendinosis/tendinopathy, and rotator cuff tears.

    This is what most “rotator cuff” content online is actually about, and
    it’s the focus of the next section.

Two extra wrinkles the videos never mention: pain from your
neck can refer into the shoulder and arm
and be mistaken for a
shoulder problem entirely (see red flags), and
it’s entirely possible to have more than one of the above at once (a
stiff and weak shoulder, for example). Sorting this out
reliably from home is genuinely hard — which is exactly what a proper
assessment is for.


Can a rotator cuff tear heal without
surgery?

This is the question behind almost every popular shoulder video, and
it deserves a straight answer rather than a hook.

The honest short version: for most rotator cuff tears,
exercise-based physiotherapy is a genuinely strong first option — often
as effective as surgery for pain and function. But “heal” needs
unpacking, and surgery is sometimes the right call.

What the evidence actually shows:

  • Rotator cuff tears are extremely common in people with no
    pain at all
    , and become more common with age. A widely cited
    finding is that roughly 48% of full-thickness tears studied were
    asymptomatic
    — the person had a genuine tear on imaging and
    felt nothing (Hinsley et al., 2022). This is why a tear showing up on
    your scan doesn’t automatically explain your pain, and doesn’t
    automatically mean you need surgery.
  • For many tears — including some large ones — exercise
    therapy produces outcomes for pain, function and quality of life
    comparable to surgery
    (Fahy et al., 2022). Rotator cuff tears
    behave more like a hole in a piece of interconnected cloth than a rope
    cut clean through: the surrounding tissue often continues to function
    well around the tear.
  • Tear size and activity level are not tightly linked to
    getting worse.
    Being appropriately active does not reliably
    make a tear larger, and a slightly larger tear on a follow-up scan
    doesn’t necessarily mean worse pain or function.
  • The honest caveat on “heal”: for most people,
    exercise improves pain and function substantially — which, for
    daily life, sport and sleep, is what actually matters. It does
    not reliably make the tear close up structurally on a
    repeat scan. If a video promises your tear will “heal” in the sense of
    disappearing from imaging, that claim is not well supported. What
    genuinely changes is how sensitive and capable your shoulder is — which
    for the overwhelming majority of people is the outcome that
    matters.

When surgery genuinely is the right call — the part the
“avoid surgery” videos skip:

  • An acute, traumatic tear (a fall, a forceful pull)
    in a younger or highly active person, especially with
    significant, sudden weakness (real difficulty lifting
    the arm, not just pain-limited effort).
  • True functional loss that a structured conservative
    programme hasn’t improved after roughly 3 to 6 months
    of genuine effort.
  • Certain tear patterns and patient goals where your surgeon and
    physiotherapist agree surgery gives the best odds — this is a shared
    decision, not a formula.

Always consult your doctor or surgeon before deciding, particularly
after real trauma. The honest position — and the one the evidence
supports — is: exercise first, for most people, with a real
trial and a real timeline
, not “surgery never” or “surgery
always.”


What you can safely do at home

No gatekeeping — several things the popular videos teach are
genuinely good starting points for a painful, non-traumatic
shoulder:

  1. Isometric holds are a good place to start if
    movement is currently painful — pressing your arm against a doorframe or
    wall in a fixed position (no joint movement) for 20–40 seconds, in
    internal rotation, external rotation and abduction. These can reduce
    pain without aggravating an irritable shoulder.
  2. The pendulum exercise — leaning over, letting the
    arm hang and relax, and gently swinging or circling it — can ease
    pressure and pain, and takes seconds to do several times a day.
  3. Scapular (shoulder-blade) squeezes — gently pulling
    the shoulder blades together and holding for 1–2 seconds, repeated —
    support posture and activate the muscles that stabilise the shoulder. A
    good habit for anyone at a desk.
  4. Gentle assisted range of motion, such as sliding
    your arms along a wall or table, or moving within a pain-free range
    using a stick or dowel — stop before pain, not at pain.
  5. Sleep positioning genuinely helps. A pillow
    supporting the arm (back sleepers) or tucked under the arm and chest
    (side sleepers, on the unaffected side) reduces the load
    pulling on an irritable shoulder overnight.
  6. Once the acute irritability settles, gradual loading matters
    more than any single stretch
    — progressing from isometrics to
    banded external rotation to light overhead work, dosed to what stays
    comfortable the next day.

The honest boundary: if pain isn’t trending better
after a genuine few weeks of sensible self-care, if you can’t lift your
arm away from your body at all, or if this followed real trauma — that’s
the point where self-management should hand over to assessment.


Frozen shoulder: a different problem, a
different timeline

Frozen shoulder (adhesive capsulitis) deserves its own section
because it is genuinely not the same problem as a rotator cuff tear, and
treating it like one doesn’t work.

What it actually is: a progressive loss of
both active and passive shoulder movement, most notably a loss
of passive external rotation of more than half compared with the other
shoulder (or less than roughly 30°), plus meaningful restriction in at
least two other movement directions. It is more common between roughly
ages 40–65, in women, and in people with diabetes or thyroid disease, or
a previous frozen shoulder on the other side.

It is not purely “adhesions to break up.” That was
the original theory (hence “adhesive capsulitis”), and it led to
aggressive stretching approaches. More recent evidence shows that under
general anaesthesia, passive range of motion can improve significantly
in frozen shoulder — meaning muscle guarding and pain are major
contributors
to the stiffness, not just physical scarring. This
matters practically: forcing an aggressively painful stretch is not the
answer, and gentler, symptom-guided treatment is.

Honest timeline — and why “quick fix” claims deserve real
skepticism:
frozen shoulder has traditionally been described in
three stages (painful “freezing,” stiff “frozen,” recovering “thawing”)
lasting many months to a few years in total. But the evidence for a
reliable, predictable natural-history timeline is weak (Wong et al.,
2017), and some people report mild-to-moderate symptoms
years after onset. If someone tells you a
treatment immediately fixed their frozen shoulder, they most likely did
not have frozen shoulder
— this is a genuinely slow-moving
condition, and that is the single most useful thing to know before
starting treatment.

How treatment is actually chosen: not by a fixed
protocol, but by how irritable the shoulder currently is — how
much pain, whether it’s present at rest or at night, and how much active
movement lags behind passive movement. A high-irritability shoulder gets
gentle, largely pain-free range-of-motion work and isometrics; as
irritability drops, exercises progress to more range, more load, and
eventually strengthening. This irritability-based approach — rather than
a generic timeline — is exactly how the method described below works.

Other treatment options, honestly appraised: a
corticosteroid injection can meaningfully reduce pain in the short term,
particularly useful early in a highly irritable phase (Kelley et al.,
2009), though evidence for a lasting difference versus other care is
less clear long-term. Manipulation under anaesthesia and surgical
capsular release exist for persistent cases and, in trial evidence,
perform similarly to well-delivered physiotherapy over a year — none has
been shown clinically superior to the others (UK FROST; Rangan et al.,
2020). These are options to discuss with your doctor, not a hierarchy
where one is obviously “best.”


What the videos don’t tell you: red flags before you
self-treat

The most-viewed shoulder videos — millions of views between them —
contain no screening for the presentations that need a doctor,
not a stretch routine.
Here it is plainly:

See a doctor promptly if shoulder pain comes with any of
these:

  • Real trauma (a fall, a direct blow, a forceful
    pull) followed by an inability to move the arm normally, visible
    deformity, or severe pain — possible fracture or dislocation.
  • Sudden, significant weakness — real difficulty
    lifting or holding the arm up, not just pain-limited effort (sometimes
    called pseudoparalysis) — this changes the urgency and the likely
    diagnosis.
  • Fever, redness, warmth or significant swelling
    around the joint, especially with feeling generally unwell — possible
    joint infection, which needs prompt medical attention.
  • Unrelenting night pain that doesn’t ease in any
    position
    , combined with unexplained weight loss or a cancer
    history.
  • Chest pain, shortness of breath, jaw or arm pain, sweating
    or nausea alongside shoulder discomfort
    — shoulder pain,
    especially on the left, can occasionally be a referred cardiac symptom.
    If in doubt, treat this as an emergency.
  • Neck pain, or numbness, tingling or weakness travelling down
    the arm into the hand
    — the pain may be coming from your
    cervical spine, not your shoulder at all, and needs a different

These are uncommon. Most shoulder pain is musculoskeletal and
responds well to exercise-based care. But screening for them is the
first thing a proper assessment does — and it’s the single biggest gap
between a follow-along video and professional care.


Myth-check: popping joints, contradictory advice, and
“damaging” a tear

Correcting these accurately matters more than being diplomatic about
them. No channels named — the ideas are what matter.

  • “You need to feel or hear a pop for the stretch to be
    working.”
    No. An audible pop or crack during a shoulder stretch
    is gas releasing in the joint (cavitation) — a side effect, not a
    treatment goal. Effective mobility work doesn’t require it, and chasing
    a pop can encourage forcing a stretch further than is useful.
  • “Exercising a torn rotator cuff will make the tear
    worse.”
    Generally not, within a sensible, symptom-guided
    programme. Tear progression is not reliably linked to appropriate
    activity levels, and controlled loading is how the surrounding muscles
    compensate and how pain and function actually improve.
  • “Popular videos disagree on whether to use weight for
    external rotation — which one is right?”
    Honestly: it depends
    on your irritability level and diagnosis, which is exactly the variable
    a video can’t know about you. Unweighted, gentle rotation is appropriate
    very early or when highly irritable; progressing to light resistance is
    appropriate as things settle. A blanket rule in either direction,
    applied to everyone, is the actual mistake — not either exercise
    itself.
  • “A pinched location tells you exactly which muscle is
    torn.”
    Pain location gives a rough clue, not a diagnosis —
    referral patterns in the shoulder overlap significantly between
    structures, which is why a clinical exam (and, where useful, diagnostic
    ultrasound) does more than pain-mapping ever can.

Your first consultation: we diagnose, we test, we
adapt

No mystery, no generic list applied blind:

  1. History and your goals. How it started (or didn’t —
    many shoulder problems build up gradually), what movements provoke it,
    how it affects sleep, work and sport.
  2. Functional and movement diagnosis. Range of motion
    (active and passive — the key distinguishing test for frozen
    shoulder), strength testing, and screening for cervical referral. Where
    it adds real value, diagnostic ultrasound is used
    in-clinic to look directly at the rotator cuff tendons and surrounding
    tissue — genuinely useful for a structure this imaging-accessible. If
    you already have an MRI or ultrasound report, we use it
    alongside the functional picture, not instead of it — a scan
    finding doesn’t automatically explain your symptoms (see above).
  3. A test treatment, same session, matched to how
    irritable your shoulder currently is — the same irritability-based logic
    used for frozen shoulder above applies broadly. We probe our first
    hypothesis and see how your shoulder responds; that response steers the
    next session. We diagnose, we test, we adapt.

Depending on findings, the path differs: an acute, irritable
shoulder
gets a gentler, pain-guided start; a stable or
recurring problem
gets a function-first plan aimed at cutting
relapse risk and building capacity for whatever you need your shoulder
to do — a desk job, a racket sport, lifting a child, swimming. In both
cases, the recommended path combines manual therapy (integrated
with osteopathic techniques) + specific, progressive exercise +
evidence-based practice
, aiming at a defined endpoint, not
dependency.


When shoulder pain has become chronic

If shoulder pain has persisted for more than three months despite
reasonable treatment, it’s rarely a simple tissue problem anymore — the
nervous system itself becomes part of the picture, shaped by sleep,
stress, activity levels and beliefs about the shoulder. That’s not “it’s
in your head” — it’s how persistent pain biology works, and it’s
precisely Philippe’s research field (PhD in chronic
pain and lifestyle interventions, VUB).

Chronic shoulder pain here is treated with the full toolkit: hands-on
treatment and graded loading to reduce sensitivity and rebuild capacity,
plus attention to the cognitive and emotional factors
that can quietly maintain a chronic situation. Few clinics can offer a
manual therapist who actively researches this.

Frequently asked questions

Can a rotator cuff tear heal without surgery? For
most people, yes in the sense that matters most — exercise-based
physiotherapy typically brings pain and function to a level comparable
with surgery, even for some larger tears. It rarely means the tear
structurally closes on a repeat scan; what changes is how your shoulder
feels and performs, which for most people is the actual goal. Surgery
remains the right choice for some situations — significant sudden
weakness after real trauma, or no meaningful progress after a genuine
3–6 month conservative trial.

Is my shoulder pain a rotator cuff tear, tendinopathy, or
frozen shoulder?
They’re different problems with different
treatment. Frozen shoulder specifically restricts both active
and passive movement; tendinopathy/impingement is usually more about
pain with certain movements and positions, with less overall stiffness.
Reliably telling them apart takes a clinical exam, not a symptom
checklist.

Will exercising a torn rotator cuff make the tear
worse?
Generally not, within an appropriately dosed,
symptom-guided programme. Tear size isn’t reliably linked to activity
level, and controlled loading is how pain and function improve.

I have shoulder pain sleeping on my side — what can I
do?
Support the arm with a pillow (under it if you’re a back
sleeper, tucked between your arm and chest if you sleep on the
unaffected side), and avoid lying directly on the painful shoulder. It
genuinely helps most people, though it treats the symptom, not the
cause.

How long does frozen shoulder take to get better?
Honestly, it’s variable and can take many months to a couple of years to
substantially resolve, and some people notice mild symptoms for even
longer. Be cautious of anyone promising a quick fix — frozen shoulder is
a slow-moving condition, and progress is best tracked over weeks to
months, not days.

Is the popping/cracking sound in my shoulder a sign the
stretch is working?
No. It’s gas releasing in the joint
(cavitation) — a harmless side effect, not a treatment goal. You don’t
need to chase it, and forcing a stretch to make it happen isn’t
useful.

When is shoulder pain serious? See a doctor promptly
if it follows real trauma with loss of arm movement, comes with sudden
significant weakness, fever or joint swelling, unrelenting night pain
with weight loss, or if it’s accompanied by chest pain, shortness of
breath or sweating (possible cardiac referral). Also worth an assessment
if it’s really coming from your neck — numbness, tingling or weakness
down the arm is a clue.

What happens in the first consultation? Your history
and goals; a functional and movement diagnosis (active and passive
range, strength, cervical screening, with diagnostic ultrasound where it
adds value); and a test treatment matched to how irritable your shoulder
